Diagnosis
You tried to enter &v1& as the person affected. But accident &v2&already has an entry for the person affected and the personnel numberis the same.
You can only enter someone once in the role of the person affected inan accident.

Procedure
Either change the role of the person or change the personnel number sothat you avoid duplicate entries.
